Home Health Telemonitoring

Home health telemonitoring uses innovative technology to collect vital signs and monitor health status from the comfort of home. It allows detection of health problems before they become serious issues which could require emergency care or hospitalization.
The in-home system guides the client through a simple procedure to collect vital signs (heart rate, blood pressure, blood oxygen level, weight and temperature, with additional option of blood glucose level) and asks condition-related “yes” or “no” questions that are easily answered with the push of a button. The monitor transmits the data via telephone to our office where it is reviewed daily by our nurse. If any of the results indicate a problem or potential problem, our nurse will call, and then take necessary action depending on the situation. This may include a visit from the nurse, a medication change or a visit to the doctor. The data can be incorporated into trend reports and provided to one's doctor on a regular basis.
Our clients may feel secure and their families enjoy peace of mind knowing that a nurse is checking on them daily and will intervene if necessary.
Our telemonitoring service is part of our standard of care for clients in our home health care program. This service is also available via private pay. We provide home health telemonitoring in Franklin, Adams and southern Cumberland Counties in Pennsylvania and Washington, Carroll and northern Frederick Counties in Maryland.
